Prospective Assessment of a Protocol Using Neuromonitoring, Early Limb Reperfusion and Selective Temporary Aneurysm Sac Perfusion to Prevent Spinal Cord Injury During Fenestrated-Branched Endovascular Aortic Repair

A prospective, single-center, nonrandomized trial from November 2013 to February 2018. spinal cord injury (SCI) prevention protocol was used for 170 patients with thoracoabdominal aortic aneurysms (TAAAs) or complex abdominal who were treated by fenestrated-branched endovascular repair (F-BEVAR). Of the patients, 91 (55%) had neuromonitoring changes, which improved maneuvers in all except 9 (10%), then underwent temporary aneurysm sac perfusion. SCI developed significantly more often a decline findings (n = [10%]) than 79 normal (1 [1%]). Permanent paraplegia occurred only 2 (1%). This standardized strategy prevent associated permanent 1% of F-BEVAR >5 cm supraceliac coverage.
